KQ Collective ("we", "us", "our") is committed to protecting the privacy of all individuals who interact with our services. As a registered life skills provider, we handle personal and sensitive information with care, in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs), and any applicable Life skills-specific obligations.

This policy explains what information we collect, how we use it, and your rights in relation to it.

What information we collect

We may collect the following types of information:

• Personal details — your name, date of birth, address, phone number, and email address
• Life skills information — your Participant number, plan details, funding categories, and support goals
• Health and disability information — only where directly relevant to delivering safe and effective support
• Emergency contact details
• Financial information necessary for invoicing and invoice claims
• Communications — records of messages, emails, or notes from meetings and phone calls

We collect information directly from you, or in some cases from your nominee, guardian, or support coordinator with your consent.

How we use your information

We use your personal information to:

• Deliver supports as outlined in your service agreement
• Submit invoice claims on your behalf
• Communicate with you about your services, appointments, and events
• Comply with our legal and regulatory obligations as a registered provider
• Improve the quality and safety of our services

We will not use your information for marketing or unrelated purposes without your explicit consent.

Who we share your information with

We may share your information with:

• The National Disability Insurance Agency (NDIA), as required for claims and compliance
• Your nominated support coordinator or plan manager, where applicable
• Other support providers involved in your care, with your consent
• Relevant health or emergency services where necessary to protect your safety
• Regulatory bodies or courts if required by law

We do not sell your personal information to third parties.

How we store and protect your information

Your information is stored securely and access is restricted to those who need it to perform their role. We use reasonable technical and organisational measures to protect your information from unauthorised access, loss, or disclosure.

We retain records for as long as required by law and our obligations as an life skills provider, after which they are securely destroyed.

Your rights

You have the right to:

• Access the personal information we hold about you
• Request corrections to inaccurate or incomplete information
• Lodge a complaint about how your information has been handled
• Withdraw consent for certain uses of your information (where this does not affect service delivery)
